Abstract

転移とは病名であり臨床的知見は大きな価値をもつ.画像診断は転移の空間的情報をもたらすと同時に,その質的な所見は原発巣だけでなく転移巣における組織病理学的知見を支持する.がんの進行は血液・血清学的な変化をもたらし,臨床的に治療の有効性や経過の判断に重要である.この変化する分子や細胞の動態こそが,原発巣が遠隔操作の媒体として遠隔臓器にがん細胞を移動させる手段と考えられる.臨床像に類似性の高い実験動物を利用した転移モデルは存在しないため,樹立された複数の人工的転移系で得られた証拠をもとに多くの転移論が主張されている.われわれの視点もその1つであり,転移臓器における本来の生理的機能の恒常性維持に関与している分子プログラムが原発巣によって質的・量的変化を受けた結果であると考えている.具体的には,肺転移の場合,原発巣の産生するVEGFやTNFαによって転移臓器である肺で遊走因子S100A8やSAA3の発現が亢進する.両者は病原体センサーTLR4の内因性アゴニストであり,TLR4を発現している骨髄球系細胞やがん細胞を肺に動員させる.肺に特異的に発現し,正常状態でも間断なく到来する気道由来の微生物や化学物質に対する生体防御機構に関与すると思われるクララ細胞によってSAA3は増幅される.すなわちホメオスターシスレベルの炎症機構ががん細胞によって巧みに利用され,その変化が増幅するような結末となる.骨転移におけるTGFβも理解しやすい例であるが,他の組織・臓器でも未知の分子群が悪循環による増幅をおこし収拾のつかない拡大系として転移微小環境を形成すると推定される.
