• The GH-IGF system should be viewed like other endocrine systems, with a central, trophic hormone (GH) and a peripherally active hormone (IGF-I). • A decrease in IGF-I production, without a concomitant impairment in GH secretion, should be termed 'Primary IGF deficiency'. • IGF-I and IGFBP-3 measurements should be given precedence in the work-up of children with suspected defects of the GH-IGF axis. • The terms 'GH insensitivity' and 'IGF-I deficiency' are not identical terms. To define a disease condition in terms of its lack of response to treatment is conceptually misleading.
