Eje estrés-mastocito y regulación de la inflamación en la mucosa intestinal: desde la salud intestinal hasta el intestino irritable

Abstract

Los trastornos funcionales digestivos y el sindrome de intestino irritable, en particular, constituyen una de las causas mas habituales de consulta medica y el diagnostico mas frecuente realizado por el especialista de aparato digestivo. A pesar de su elevada prevalencia, la etiologia y fisiopatologia de los procesos funcionales digestivos no estan bien establecidas, y tampoco hay metodos diagnosticos especificos ni opciones terapeuticas claramente eficaces. Por todo ello, estos pacientes experimentan a menudo un deterioro importante de su calidad de vida que condiciona un gasto sanitario creciente. Evidencias recientes indican que la presencia de microinflamacion en la mucosa intestinal con hiperplasia mastocitaria podria contribuir al desarrollo y perpetuacion de los patrones de hipersensibilidad visceral, dismotilidad y alteraciones de la barrera epitelial que caracterizan al sindrome de intestino irritable. En este articulo revisaremos el papel del eje estres-mastocito en la modulacion de la inflamacion de la mucosa intestinal y en la patogenia del sindrome de intestino irritable.
