Infrautilización de los antiagregantes plaquetarios en pacientes con enfermedad cardiovascular

Abstract

ObjetivoConocer si hay un correcto tratamiento antiagregante en los pacientes que tienen un alto riesgo cardiovascular.DiseñoEstudio transversal.EmplazamientoHospital Álvarez Buylla, Mieres, Asturias.ParticipantesUn total de 219 pacientes ingresados consecutivamente desde el 1 de enero de 2002 por un síndrome coronario agudo.Mediciones principalesSexo, diagnóstico en el momento del alta, tratamiento con antiagregantes plaquetarios antes del ingreso y antecedentes clínicos de enfermedad arteriosclerótica y diabetes.ResultadosEl 26,9% de nuestros pacientes tomaba antiagregantes plaquetarios en concreto, ácido acetilsalicílico (AAS); de ellos, sólo 4 seguían este tratamiento por causa distinta del riesgo cardiovascular. No encontramos diferencias significativas por sexos. De los pacientes que tenían indicación de tratamiento antiagregante por enfermedad arteriosclerótica (antecedentes personales de cardiopatía isquémica, accidente cerebrovascular isquémico [ACV], isquemia crónica de miembros inferiores [ICMI]) o riesgo cardiovascular elevado (diabéticos), únicamente el 42,5% recibía tratamiento antiagregante (p<0,005). En el caso de los diabéticos, sólo lo recibía el 37,5% (p=0,025). De los pacientes con cardiopatía isquémica, recibía tratamiento el 56% (p=0,005), mientras que de los que tenían ICMI, lo recibían el 23,1% (p=NS) y el 21,7% (p=NS) de los que habían presentado un ACV.El porcentaje de pacientes con angina que tomaba AAS fue mayor que en los infartos, con un 40,7% de pacientes con angina inestable y un 21,9% en los infartos (p=0,023). El riesgo relativo de presentar un infarto en la cohorte que tomaba AAS fue de 0,408 (intervalo de confianza [IC] del 95%, 0,215-0,775). Al separar por sexos, esto sólo se mantiene en los varones (tabla 1), con una odds ratio (OR) de 0,318 (IC del 95%, 0,151-0,668; p=0,002).Discusión y conclusionesLimitaciones: puede haberse subestimado el porcentaje de pacientes que no reciben tratamiento antiagregante con un riesgo cardiovascular elevado al considerar sólo los ingresos hospitalarios por síndrome coronario agudo. Superamos los datos de algunos estudios1,2 en el porcentaje de tratamiento con AAS en cardiopatía isquémica, a pesar de ser bajo y menor que en el PEPA3 o el PRESENTE4, que superaban el 90%. Encontramos, al igual que en otros estudios, que la toma de AAS disminuye el riesgo de presentar un evento cardiovascular más grave5,6. Esto parece más marcado en varones, pero son necesarios más estudios para determinar la causa de este mayor beneficio en varones.Hay una infrautilización de los antiagregantes en pacientes con arteriosclerosis diagnosticada. Los varones se benefician más del tratamiento antiagregante.
