Transmisión del virus de la hepatitis C en gestantes

Abstract

Fundamento y objetivo Conocer la prevalencia de infeccion por el virus de la hepatitis C (VHC)en mujeres embarazadas asturianas y el porcentaje de transmision vertical de las mismas. Pacientes y metodo Estudio longitudinal prospectivo realizado entre el 1 de septiembre de1997 y el 31 de diciembre de 1998 en una serie consecutiva de gestantes del Area Sanitaria Vdel Principado de Asturias. Inicialmente se calculo la prevalencia de la infeccion mediante serologiaen el primer trimestre y el porcentaje de transmision de madre a hijo de las madres positivas.Se realizo un seguimiento de los hijos de madres infectadas hasta los 18-24 meses deedad. Se determinaron los anticuerpos anti-VHC en todas las gestantes. A las gestantes positivasse les determino el ARN del VHC por reaccion en cadena de la polimerasa (PCR), el genotipoy la carga viral al parto. A sus hijos se les determinaron anticuerpos anti-VHC y ARN delVHC al nacimiento y a los 3, 6, 12 y 18 meses. Resultados De las 2.442 gestantes consecutivas estudiadas, 30 presentaban anticuerpos frenteal VHC (prevalencia infeccion por VHC: 1,23% de las gestaciones y 1,20% de las mujeres);de ellas, 24 eran positivas por PCR al ARN del VHC. De las 30 madres nacieron 30 ninos conanticuerpos frente al VHC, de los que 29 aclararon sus anticuerpos y uno quedo finalmente infectado(transmision vertical 3,3%). El nino infectado era hijo de madre coinfectada por el virusde la inmunodeficiencia humana (VIH) y el VHC, su genotipo VHC era tipo 1b y la carga viralmaterna al parto era 9,6 × 105 copias/ml. Conclusiones En nuestro entorno, la prevalencia de la infeccion por el virus de la hepatitis C esdel 1,20% de las mujeres embarazadas. La transmision vertical entre madres infectadas por elvirus de la hepatitis C y no seleccionadas previamente es del 3,3%.
