The routine lumbar injection of 131I human serum albumin (RISA) may be used instead of the more difficult and potentially dangerous intraventricular, cisternal, or direct shunt injections to determine the patency of ventricular shunts in patients with communicating hydrocephalus. In a hydrocephalic with a functioning shunt the path of least resistance is through the shunt via the ventricle with activity localizing in the ventricle within six hours. A false diagnosis of patent shunt could result if there were a complete subarachnoid block above the tentorium.
