A distinctive urographic finding was discovered in a young patient with hematocolpos. The case is reported to familiarize the radiologist with this unusual cause of uterine enlargement and ureteral deviation. A 14-year-old white girl was admitted in moderate distress, with an enlarged abdomen and a bulging vaginal mass, but no specific urinary complaints. No menstrual flow had occurred as far as the patient or her mother knew. Physical examination, except for the mass, was not remarkable. Urinalysis was negative. There was no evidence of congenital anomaly except for an imperforate hymen. On urography bilateral hydronephrosis and hy-droureter were demonstrated, with a lateral deviation of the ureters, particularly above the brim of the pelvis where they took a sharp upward and lateral course.
