In a review of 48 cases, pediatric pericardial abnormalities were categorized by etiology: developmental, tumor-related, pericarditic, and postsurgical. In 34 (70%) a pericardial effusion was present at some point in the clinical course; the plain film in these cases revealed that straightening of the left heart border is an early sign of fluid within the pericardial sac and appears before the classic globular heart. Fluoroscopy and radionuclide and ultrasound studies can help confirm the plain film impression. In addition, the various pericardial abnormalities are discussed with reference to the literature.
