Furthermilore, therisks oftheactual operation andofpost-operative comlplications, thefrequent occurrence ofdivergence, andthepossible disappointment ofthepatient atthefinalresult, foster this conservative attitude. Cowan(1949) states: Thecorrection ofmonocular aphakia isimpracticable, notonlybecause ofthe diplopia duetoaniseikonia andtotheprismatic effect ofthedifference between theglasses, butalsobecause thepatient hastwo different optical instruments, one dynamic withmore or lessactive accommodation andtheotherstatic. Itisgtener11ally agreed that(tlhere bDeillg goodvisual acnitv in thefello\- eye)thecorrection ofunilateral wapakia withspec(tacle lenses isdi-sapll)oinatin lg, becatise oftheaniseikonia aind(liplopia. Incontraffiction oftlisview\, KulhnandKluhtln (1949) advise normalspectacle lenlses and report a series of8Xiuniocular aphakias corrected inthisxway. Theirresults ar-estar-tlilng: 83percent. ofcaseshadnotrouble lwhatsoever inwearincg full correction; onlv4perccnt. wxere totally unable towearfullcorrection; andtherenmaining 13percent. hadmoderate trouble for a fewmonths.Thepatients werefitted witha temporarv lens 4to6weeksafter theoperation andanoccluder waswornwhile thediplopia lasted. Although nodetails aboutbinocular vision aregiven, theystate that " somepersons suppress oneimagebut milost ofthemactually lhave binocular vision.Hardasitisto understand Mostauthors agreethataconltact lens, atleast theoretically, is thebestremedywe haveatpresent forabolishing thediplopi andcaniseikonia wlichaccompanytheuseofspectacle lenses. According toTown(1939), thediplopia ischiefly duetotheprismaticeffect ofspectacle lenses ofhighpositiv e powerandnot tc)aniseikonia, asisgenerally believed. Thecontact lenswill abolislh tlisprismatic effect, andthediplopia thatmay occur withacontact lensisprobably duetomuscleimbalance.
