Summary A 48-year-old Lebanese man presented with a 2-year history of painless papules and nodules of the skin without any systematic involvement, lymphadenopathy, serum electrophoresis disturbance, leukocytosis or elevated erythrocyte sedimentation rate (ESR). The microscopic appearance was highly suggestive of sinus histiocytosis with numerous plasma cells and intense lymphophagocytosis. Rapid improvement occurred following thalidomide therapy: 300 mg a day for a month.
