We describe a rare case of scalp hair repigmentation following radiotherapy in a 76-year-old man. He was treated with 45 Gy over 25 fractions for squamous cell carcinoma of the scalp and neck, and after a period of radiation-related alopecia, his hair grew back with the orange–brown pigment of his youth. This is a rare phenomenon, with this report adding to the limited literature and exploring how radiation may influence melanocyte stem cell plasticity and scalp hair pigmentation.
