A 33-year-old woman presented with a painless swelling inthe right submandibular region that had been enlarging pro-gressively over the past 1 and 1/2 years. There was no historyof local trauma or infection, and the area did not changeduring meals. The medical and family histories were noncon-tributory.Physical examination revealed a soft, movable, poorly cir-cumscribed swelling measuring 5 5 cm in the right sub-mandibular area (Fig 1). The overlying skin appeared normaland did not appear to blanch significantly with compressionof the mass. No bruit or thrill was detected. Intraorally, thefloor of the mouth was nonelevated with a normal surfacemucosa. Bimanual palpation elicited equivalent submandibu-lar salivary flow from both the right and left sides.Initial panoramic and mandibular occlusal radiographsfailed to disclose any abnormal findings. On subsequent ul-trasound examination, the left submandibular gland appearedto be of normal size and echogenicity. A 4.5 1.7-cm masswith well-defined borders was detected on the lateral andmedial aspects of the right submandibular gland. The massdemonstrated lower echogenicity and a homogenous patternsuggestive of fluid content (Fig 2).Computed tomography (CT) with contrast demonstrated alarge, well-circumscribed mass measuring 4.5 3 2cminthe submandibular region, extending laterally and mediallyfrom the angle of the mandible to the submandibular gland.The homogeneous mass appeared attached to the gland andlacked evidence of calcification (Fig 3).
