As complementary and alternative medical (CAM) therapies increasingly penetrate conventional health care, physicians and other health care providers find themselves providing (or being asked to provide) care at the borderland of medicine, ethics, public policy and law. Whereas historically, many physicians and patients shunned a dialogue concerning use and potential applicability of CAM therapies in mainstream care, such dialogue is now flourishing, both in the US and internationally. Discussions about CAM are enriching the physician–patient encounter, educating various players in the health-care arena about the role of nutrition, herbal medicine, mind-body interactions, and other phenomena in mainstream care and contributing to a greater foundation of world medical knowledge. Such dialogue is also leading physicians to reconsider their moral, ethical and legal obligation to be aware of the best available evidence emerging from CAM, to present such evidence to the patient in meaningful terms and to confront therapeutic issues and choices from a more comprehensive, unified perspective on health and the possibility for human healing. This chapter explores and outlines the regulatory boundaries of such an obligation.
