Lung transplantation in children is an effective treatment for end-stage pulmonary disease after all medical therapy has failed. It requires a huge investment in resources and absolute commitment on the part of the parents and patients. In spite of all these efforts and expense, the results with pediatric lung transplantation are the worst of all solid organ transplants. Much lies ahead to overcome the obstacles faced to improve this therapy.
