Abstract

Resumen La sintomatologia miccional secundaria a hiperplasia benigna de prostata (HBP) y la disfuncion erectil son 2 entidades altamente prevalentes en el varon a partir de los 50 anos. Ambas entidades tienen un impacto negativo sobre la calidad de vida. La finalidad de este articulo es hacer una revision acerca de la relacion que hay entre la sintomatologia miccional y la disfuncion erectil, asi como de las posibilidades de tratamiento con inhibidores de la fosfodiesterasa 5. Hay estudios epidemiologicos que establecen una asociacion entre la sintomatologia del tracto urinario inferior secundaria a HBP y la disfuncion erectil, de tal forma que la sintomatologia miccional es un factor de riesgo para la disfuncion erectil. Por otra parte, hay datos que muestran bases fisiopatologicas comunes entre ambas entidades. Entre las hipotesis patogenicas existentes destaca la que establece la via del oxido nitrico-GMPc como mecanismo fisiopatologico comun para la disfuncion erectil y la sintomatologia urinaria. Hay estudios con cada uno de los inhibidores de la fosfodiesterasa 5 comercializados que indican que pueden ser farmacos eficaces en el tratamiento de la sintomatologia del tracto urinario inferior, asociada o no a la disfuncion erectil. Ademas, se pueden utilizar solos o en combinacion con los bloqueadores alfa. Tambien hay estudios preliminares que apuntan la posibilidad que los inhibidores de la fosfodiesterasa 5 puedan ser utiles en el tratamiento de la vejiga hiperactiva. Se deben realizar estudios mas amplios para establecer la verdadera utilidad de los inhibidores de la fosfodiesterasa 5 en el tratamiento de la sintomatologia del tracto urinario inferior, sola o asociada a disfuncion erectil.
