Placebo y efecto placebo

Abstract

Placebo significa «medicamento» sin principio activo, capaz de producir un efecto. El efecto placebo se produce en sintomas funcionales donde la subjetividad es muy importante. Las hipotesis responsables del efecto placebo son psicologicas y neurobiologicas, estas ultimas basadas en estudios de imagen funcional cerebral. El sistema dopaminergico y el sistema opioide estan implicados en el efecto placebo. Con excepcion del coma y las enfermedades infecciosas, numerosas afecciones o sintomas cronicos parecen ser sensibles al efecto placebo. La manera de emplear el placebo como tratamiento es precisa, especialmente la informacion facilitada al paciente sobre lo que es un placebo debe ser clara, y la indicacion de su prescripcion debe estar argumentada. En los ensayos clinicos, el placebo permite garantizar la validez cientifica de los resultados obtenidos, lo que posibilita conseguir el mayor nivel de evidencia, pero debe descartarse su uso en monoterapia si existe un tratamiento de referencia validado. Es fundamental, en todos los casos, que su uso respete el principio del consentimiento informado.
