Tratamiento anticoagulante durante el embarazo

Abstract

La administracion de tratamientos anticoagulantes durante el embarazo se reserva para la prevencion y el tratamiento de la enfermedad tromboembolica venosa (ETEV) y las enfermedades vasculares placentarias (EVP): preeclampsia y sus complicaciones, retraso del crecimiento intrauterino, abortos espontaneos de repeticion y muerte fetal in utero. Estas enfermedades son multifactoriales, pero la simple anamnesis resulta suficiente para identificar sus factores de riesgo, lo cual es primordial para su prevencion. Los principales factores de riesgo clinicos son los antecedentes personales y familiares. Las trombofilias tambien constituyen un riesgo variable en funcion de la anomalia de la ETEV, pero su participacion en la EVP es mas incierta. Las heparinas de bajo peso molecular han sustituido a las heparinas no fraccionadas y constituyen el tratamiento de eleccion de la ETEV durante el embarazo. Dado que no atraviesan la barrera placentaria, las heparinas de bajo peso molecular son seguras para el feto, a diferencia de las antivitaminas K y los nuevos anticoagulantes orales. Estos ultimos estan, por lo tanto, formalmente contraindicados. La aspirina a dosis bajas es actualmente el unico tratamiento que ha demostrado resultar beneficioso para la prevencion de las EVP. El tratamiento se basa en la aplicacion de las recomendaciones. La uniformizacion de los tratamientos es fundamental para ello; la utilizacion de una estratificacion del riesgo tromboembolico y de enfermedad vascular placentaria es deseable y susceptible de mejorar la morbimortalidad, al limitar los excesos y carencias terapeuticas en estas pacientes.
