Psicosis en la adolescencia

Abstract

Resumen Las psicosis constituyen, sin duda, la principal preocupacion de los psiquiatras especialistas en adolescentes y la obsesion de sus observaciones clinicas. La adolescencia plantea, en efecto, de manera crucial, dificiles cuestiones diagnosticas ante sintomas llamativos o mas discretos que expresan de modo polimorfo el sufrimiento psiquico frecuente en estas edades, sin que por ello exista de forma obligatoria una correlacion estrecha con un trastorno mental demostrado. Etapa de profundas mutaciones internas y externas, la adolescencia tambien es la edad preferente de aparicion de la mayoria de las enfermedades psiquiatricas potencialmente cronicas, que al principio no suelen tener las caracteristicas clinicas que mas tarde las hacen reconocibles. Por tanto, en la adolescencia, mas que en cualquier otro periodo de la vida, el clinico debe sobrepasar el nivel de descripcion nosografica para investigar la dimension psicopatologica de los sintomas que observa, asi como su valor diagnostico y pronostico, con un temor principal: el inicio de una esquizofrenia, cuadro que presenta preferencia por esta edad. Tampoco debe olvidarse la frecuencia de trastornos del animo que tambien aparecen en esta epoca, a menudo con caracteristicas psicoticas enganosas. Para interrogarse sobre las psicosis de la adolescencia, hay que plantearse asimismo el valor de las manifestaciones premorbidas que aparecen en esta etapa de la vida: factores de vulnerabilidad o prodromos de la enfermedad con modos de tratamiento o de prevencion que, como consecuencia, pueden ser muy diferentes.
