Abstract

Varon de 89 anos, con antecedentes personales de enfermedad de Alzheimer, hipertension arterial y enfermedad pulmonar obstructiva cronica, que acude a consulta por presentar una lesion en labio inferior de mas de 5 anos de evolucion. Fue intervenido de un carcinoma basocelular (CBC) de pequeno tamano en la comisura de labio inferior en 2005. La lesion recidivo y se planteo tratamiento quirurgico que el paciente rechazo. La tumoracion ha ido creciendo de forma progresiva (fig. 1). En la exploracion, se aprecia una tumoracion de mas de 8 cm, indurada, nodular, con algunas telangiectasias, costras hemorragicas y borde perlado (por tener micronodulos translucidos en sus margenes). La lesion ha invadido la mucosa del tejido labial inferior. No refiere dolor ni se palpan adenopatias. Ante esta manifestacion cutanea, considerando la morfologia de las lesiones, se plantea como diagnostico principal un CBC en estadio avanzado. Se deriva de forma preferente a dermatologia, donde se confirma el diagnostico y se programa la intervencion quirurgica. Figura 1. Deriva de celulas pluripotenciales de la capa basal de la epidermis o del epitelio folicular. Se da con mayor frecuencia en personas mayores de 50 anos, de piel clara, que han estado sometidas a exposiciones solares inter mitentes e intensas1,2.
