Myosite à Mycobacterium chelonae

Abstract

Resume Introduction Les mycobacteries sont essentiellement responsables d’infection de la peau et des tissus mous. Une myosite a Mycobacterium chelonae est survenue chez un patient immunodeprime. Observation Un homme âge de 49 ans, traite au long cours par corticoides pour une vascularite des membres inferieurs associee a une polyarthrite rhumatoide, a ete hospitalise pour aspergillose pulmonaire invasive. Dix jours plus tard s’est developpee une myosite du membre superieur droit avec de multiples abces sous-cutanes. Les cultures de ces collections purulentes ont mis en evidence Mycobacterium chelonae. Un traitement par ciprofloxacine et clarithromycine a obtenu la regression des lesions; il a ete poursuivi pendant 12 mois. Commentaires M. chelonae se trouve en quantite importante dans l’environnement. L’infection par cette mycobacterie est favorisee par l’immunodepression, en particulier par celle secondaire a une corticotherapie. Les resistances aux antibiotiques sont frequentes. La clarithromycine a une excellente activite contre cette mycobacterie. Une bitherapie est recommandee pour eviter l’emergence de resistance.
