Artroplastia total de rodilla modelo TACK con platillos móviles

Abstract

Objetivo Presentar las caracteristicas tecnicas y el diseno de la artroplastia total de rodilla semiconstrenida modelo TACK (total articulating cementless knee) de LINK, asi como mostrar los resultados obtenidos en nuestro servicio, el grado de satisfaccion de los pacientes y cuantificar el numero de complicaciones. Material y metodo Presentamos un estudio retrospectivo y descriptivo sobre una revision de todas las protesis TACK implantadas en nuestro servicio durante el periodo 1993- 2001. Se incluyen un total de 92 protesis colocadas en 78 pacientes. Predominaron el sexo femenino (86,7%) y la rodilla derecha (57,6%). La edad media fue de 67,5 anos, y el seguimiento medio fue de 25,3 meses (rango 12-96 meses). Resultados Hemos evaluado los resultados obtenidos utilizando el sistema de clasificacion Knee Society Score, que valora de forma independiente, por un lado, el «estado de la rodilla» —que paso de 50,3 ± 14,2 puntos en el preoperatorio a 92,1 ± 4 puntos tras 3 o mas anos—, y por otro la «evaluacion funcional del paciente» —que paso de 43,3 ± 15,4 puntos de media en el preoperatorio a 81,7 ± 15,2 puntos a los 3 o mas anos—. A su vez valoramos el estado de satisfaccion personal del paciente siguiendo la escala modificada de Dawson, encontrando 43 casos excelentes (46,7%), 29 buenos (31,5%), 13 regulares (14,1%) y 7 malos (9,8%). Entre las complicaciones mas importantes encontramos 4 casos de infeccion profunda, 7 de movilizacion aseptica, 9 presentaron dolor en la cara anterior de la rodilla y 3 de trombosis venosa profunda. Conclusiones La protesis TACK proporciona unos resultados excelentes en pacientes con osteoartrosis degenerativa, con una indicacion precisa y una tecnica quirurgica depurada. Hemos obtenido una tasa de complicaciones similares a las de la bibliografia. Recomendamos la escala de valoracion de la AKSS asociada a una escala que valore la percepcion del paciente acerca de su protesis de rodilla.
