Abstract

Resume L'antibiotherapie (macrolides en particulier) demeure une des armes majeures pour le controle de la coqueluche. Administree tot, elle ameliore la symptomatologie clinique et reduit la contagiosite. Lorsque la phase des quintes est etablie, elle reste indiquee pour diminuer le risque de contagion. Les indications de l'antibioprophylaxie dans l'entourage d'un cas index sont larges, meme si les sujets ont ete vaccines. L'erythromycine (50 mg/kg/j en quatre prises pendant 14 jours) reste le traitement de reference, le seul ayant prouve son efficacite clinique, tant en curatif qu'en prophylactique. D'autres composes mieux toleres, appartenant a cette famille (spiramycine, josamycine, roxithromycine, clarithromycine et azithromycine), ont des caracteristiques pharmacocinetiques-phamacodynamiques laissant entrevoir une activite clinique prometteuse. Les alternatives aux macrolides sont le cotrimoxazole et les fluoroquinolones chez l'adulte.
