Abstract

Las enfermedades del tejido conectivo comprenden un amplio grupo de entidades cuya etiologia es desconocida, caracterizadas por afectar a multiples organos y sistemas, que comparten una patogenia inmunologica y dan lugar a manifestaciones de tipo inflamatorio de todo tipo, aunque su lesion basica es siempre una vasculitis difusa. Cualquier estructura cardiovascular puede verse afectada, desde el pericardio, el miocardio, el endocardio y las valvulas a las arterias coronarias, la aorta, las arterias pulmonares las arterias perifericas, venas, arteriolas, venulas y lechos capilares de casi todos los organos y sistemas. Los estudios anatomopatologicos ponen de manifiesto una alta prevalencia de afeccion cardiaca, pero existe una escasa correlacion entre los hallazgos patologicos y las manifestaciones clinicas. La incorporacion de la ecocardiografia Doppler ha permitido el reconocimiento de casos de menor entidad y mas precozmente. Aunque este tipo de pacientes continuan siendo controlados por reumatologos e internistas, la frecuente afeccion cardiaca requiere que los cardiologos conozcan sus caracteristicas, evolucion y tratamiento.
