La nariz como espejo de los bronquios

Abstract

La idea de que la nariz esta intimamente relacionada con los bronquios no es nueva, sino que ha tenido sus grandes momentos desde la era cristiana. Asi, Galeno proponia el purgar las cavidades nasales como un remedio para el asma. Desafortunadamente, su hipotesis incluia la propuesta de que el fluido nasal provenia del cerebro, de modo que la conexion narizbronquios sufrio un reves considerable al demostrarse que esto era imposible. Para el neumologo contemporaneo, la nariz ha sido con frecuencia considerada como un organo alienado de las vias aereas inferiores y considerado inmerecedor de un esfuerzo cientifico serio. Sin embargo, este prejuicio esta hoy dia desvaneciendose por razones tanto cientificas como practicas. La diferencia mas notable entre la nariz y los bronquios es anatomica. Mientras que una exhibe un diseno complejo, mas bien estrambotico, los otros se ramifican siguiendo un patron extremadamente sencillo. La organizacion anatomica confiere a la nariz funciones especificas en los procesos de calentamiento, humidificacion y filtracion del aire inhalado, con implicaciones significativas en cuanto al desarrollo de enfermedad. Por ejemplo, la naturaleza turbulenta del flujo aereo que se genera en los pasajes nasales resulta en un mayor deposito, por area, de particulas alergenicas grandes en la nariz, en comparacion con los bronquios. No es pues sorprendente que individuos alergicos al polen desarrollen primordialmente rinitis mientras los sujetos alergicos a esporas principalmente desarrollan asma.
