Concentraciones plasmáticas de fibronectina como marcador de evolución en pacientes sépticos

Abstract

Introduccion El seguimiento clinico de los pacientes septicos presenta numerosas dificultades debido a la presencia de enfermedades de base o la aparicion de complicaciones. En la practica diaria el clinico necesita conocer lo antes posible la respuesta del organismo tras la instauracion de un tratamiento antibiotico empirico. Este hecho impulsa a la busqueda constante de nuevos biomarcadores capaces de indicar o, por lo menos orientar, sobre la situacion, a tiempo real, del paciente infectado. La fibronectina es una proteina cuya concentracion plasmatica ha mostrado ser una buena herramienta en el diagnostico precoz de la sepsis; por tanto, nos planteamos su posible empleo en la monitorizacion clinica de este tipo de pacientes. Material Se disena un estudio de seguimiento de la concentracion plasmatica de fibronectina en pacientes septicos ingresados en el hospital de Segovia durante los meses de febrero a agosto de 1995. Como grupos de comparacion se preparan dos: uno constituido por controles de patologia variada y otro de controles sanos. Resultados Aplicando la prueba de comparaciones multiples de Bonferroni, se muestra que la fibronectina plasmatica se comporta como un reactante negativo de fase aguda cuya concentracion se mantiene practicamente indetectable en el tiempo mientras el paciente no mejora y que cuando el tratamiento es eficaz, dos dias despues, ya se obtienen incrementos significativos. Conclusiones La concentracion plasmatica de fibronectina como determinacion aislada o formando parte de un sistema de score podria tener utilidad en el seguimiento clinico de los pacientes septicos.
