Abstract

PUNTOS CLAVE Introduccion. La artritis reumatoide (AR) es una sinovitis cronica de naturaleza autoinmune, que afecta a personas geneticamente predispuestas. En la mayoria de los pacientes el curso es progresivo. En la actualidad se considera la causa mas comun de incapacidad en el mundo occidental, que es potencialmente tratable. Definicion de objetivos terapeuticos Induccion de la remision. Es el objetivo primordial del tratamiento de la AR, y se entiende por ello la eliminacion completa de la inflamacion articular; se obtiene mediante la administracion de los llamados farmacos modificadores de la enfermedad (FAME): metotrexato, leflunomida, sulfasalazina y los modernos agentes biologicos anti factor de necrosis tumoral (TNF): infliximab, etanercept y adalimumab. El tratamiento inductor de remision se basa en cuatro principios basicos: 1. Utilizacion eficiente de los FAME. Aumento escalonado de la dosis hasta alcanzar la remision, o substitucion por otro si no se alcanza el objetivo, de forma que se eviten tiempos muertos en el tratamiento, es decir, que en todo momento el paciente con AR este recibiendo un farmaco que tenga posibilidad de suprimir por completo la inflamacion. 2. Evaluacion sistematizada de la inflamacion articular. Es imprescindible para poder detectar grados menores de inflamacion, y verificar que el tratamiento ha conseguido sus objetivos. 3. Tratamiento precoz. Esencial a fin de alcanzar la remision antes de que se hayan producido lesiones estructurales que originen secuelas irreversibles. 4. Informacion adecuada al paciente. Este debe tener claros los objetivos del tratamiento y el nivel de mejoria que se puede alcanzar. Medidas paliativas. La mejora de los resultados obtenidos con los tratamiento inductores de remision, junto con el tratamiento precoz, ha hecho que estas medidas paliativas, frecuentemente enfocadas al tratamiento de secuelas, haya ido ocupando un lugar progresivamente mas secundario.
