Abstract

Introduction Les lesions induites par la foudre sont variees. Nous decrivons un cas d’atteinte visuelle secondaire au foudroiement chez une femme de 42 ans, documente par OCT. Objectifs et Methodes Description du decours clinique d’une perte visuelle unilaterale en deux phases apres foudroiement. Observation Une guide de montagne de 42 ans est foudroyee en altitude. Son entourage rapporte un bref episode de perte de connaissance. Quelques heures plus tard, une baisse de l’acuite symptomatique est notee a l’OD a 0.5. L’examen a la lampe a fente montre une discrete uveite anterieure. La desaturation au rouge et la pâleur papillaire suggerent une neuropathie optique retrobulbaire pour laquelle des steroides intraveineux sont administres. L’angiographie a la fluoresceine et l’IRM cerebrale sont normales. Six jours plus tard, la patiente decrit une baisse d’acuite soudaine accompagnee de metamorphopsies. L’acuite visuelle mesuree est de mouvements de la main. Le fond d’œil montre un epaississement maculaire kystique confirme a l’OCT. A 3 mois, l’acuite visuelle est inchangee. L’OCT montre une lesion kystique foveolaire de grande taille. Discussion Les sequelles ophtalmiques du foudroiement comprennent l’occlusion de la veine centrale de la retine, l’œdeme maculaire, les trous maculaires et les papillites. Notre patiente a presente une atteinte visuelle en deux phases. Une degenerescence des photorecepteurs d’origine electrique a succede aux lesions post-traumatiques immediates (iritis et neuropathie optique retrobulbaire aigue). La nature destructrice des lesions, objectivees a l’OCT, explique le mauvais decours fonctionnel. Conclusion Les photorecepteurs foveaux peuvent etre detruits lors du foudroiement. L’OCT est l’examen de choix pour documenter les lesions.
