Suivi sur 17 ans des FEIR : évolution dans un établissement MCO

Abstract

Introduction La clinique Saint-Leonard est un etablissement medicochirurgical, centre sur la prise en charge medicochirurgical de l’appareil locomoteur. Les actes chirurgicaux a risque de transfusion sont les pathologies des grosses articulations, hanche et genou (soit 600 cas/an/chacune ; fracture traumatique col du femur, soit 150 cas/an) et chirurgie complexe du rachis (arthrodeses etendues = 200 cas/an ; scoliose = 25 cas/an). Description L’etude retrospective porte sur les FEIR declarees a l’EFS d’Angers, ayant donne lieu a une FIT. Jusqu’en 2008, la transfusion est homologue (TH) et autologue (TA). En 2008, cette derniere est stoppee. La transfusion homologue (TH) diminue regulierement d’un facteur 7 depuis 2000 et d’un facteur 4 depuis 2008 ( Fig. 1 courbe evolutive transfusion). La declaration annuelle des FEIR est faible = 2,8 cas/an en moyenne, 3,6 cas/an entre 2000 et 2008 et 1,9 apres 2008, soit une baisse d’un facteur 2 ( Fig. 1 courbe declaration FEIR). Explications Deux facteurs interviennent dans la baisse des FEIR : – baisse reguliere de la TH depuis 2008 : facteur 4. L’augmentation de l’activite chirurgicale est reguliere entre 0,1 a 3 %/an ; – baisse de la TA depuis 2000 (1350 PSL en 2000) et arret en 2008. Les raisons de la baisse de la transfusion sont autant chirurgicales (techniques moins hemorragiques, prise en charge Discussion Les FEIR augmentent en rapport avec la transfusion : 0,33 % jusqu’en 2008 et 1,47 % ensuite ( Fig. 1 courbe rapport FEIR/transfusion). Avant 2008, les FEIR impliquent surtout le syndrome frissons-hyperthermie (58 %) et la positivite des RAI (21 %) et concernent la TA (80 % des FEIR) (nature FEIR). Apres 2008, l’essentiel des FEIR concerne les RAI post-transfusionnelles positives (78 %) et donc dans le cadre de la TH. Conclusion Les FEIR correspondent a un faible echantillon = 47 cas de 2000 a 2017. Elles sont liees a des problematiques de positivite des RAI, dans le cadre de la TH.
