Abstract

ResumenIntroducciónEl 95% de los gemelos monocoriales-monoamnióticos tienen anastomosis placentarias venoarteriales que permiten transfusión de sangre entre ellos. Sin tratamiento ambos bebés se ven afectados.Material y métodosSe analizó retrospectivamente el desenlace de 55 embarazos gemelares monocoriales-monoamnióticos complicados por síndrome transfusión feto-feto y tratados con coagulación láser en el Instituto Nacional de Perinatología (INPer) de la Ciudad de México, de enero 1999 a julio 2014. La información se obtuvo de los expedientes clínicos; para el análisis descriptivo se calcularon porcentajes, medias y desviaciones estándar, se utilizó la prueba «t» de Student para las comparaciones de variables cuantitativas y Chi cuadrado para las cualitativas, mediante SPSS v 20.ResultadosSe obtuvieron 48 bebés vivos, 38 óbitos, 20 continuaron tratamiento fuera del INPer. La supervivencia se relacionó con la etapa de Quintero al momento de tratamiento: etapa 1 (83%), 2 (35%), 3 (67,5%) y 4 (50%). La edad gestacional al realizarse coagulación láser fue de 23.3 semanas. El 93.7% de los productos nació por cesárea a las 31.2 semanas (56 días postratamiento); peso promedio de 1399.8g. Hubo diferencia significativa entre el hematocrito del donador (45±10.6) vs. receptor (48.9±11,2) (p<0.001). Complicaciones encontradas: hidropesía (4%), enterocolitis necrosante (4%) y trombosis (2%); las principales morbilidades fueron: retinopatía (14%), displasia broncopulmonar (10%) y alteraciones en el neurodesarrollo (14.5%).ConclusionesLa supervivencia de pacientes con síndrome de transfusión feto-feto se relaciona con la etapa Quintero y edad gestacional; por tanto, la detección y tratamiento oportuno mejora el pronóstico y la calidad de vida.AbstractIntroductionThe large majority (95%) of monochorionic-monoamniotic twins have arterio-venous placental anastomoses that allow blood to transfuse from one to the other. Without treatment, both babies are affected.Material and methodsA retrospective analysis was performed on the outcomes of 55 monochorionic-monoamniotic twin pregnancies, complicated by twin to twin transfusion syndrome, and treated with laser coagulation in the Instituto Nacional de Perinatología (INPer) in Mexico City, from January 1999 to July 2014. Data was obtained from clinical charts. Percentages, means, and standard deviations were calculated for the descriptive analysis. The Student t-test was used to compare quantitative variables, and χ2 test for qualitative ones, using SPSS v. 20 software.ResultsThere were 48 live babies, 38 stillbirths, and 20 continued their treatment outside the INPer. Survival is related to the Quintero stage at the time of treatment: stage I (83%), II (35%), III (67.5%), and IV (50%). Mean gestational age at laser coagulation was 23.3 weeks. A caesarean section was used for 93.7% of the deliveries at 31.2 weeks (56 days post-treatment). The mean birth weight was 1399.8g. There was a statistical difference in haematocrit values between donors (45±10.6) and recipients (48.9±11.2) (P<.001). Complications were: hydrops (4%), necrotising enterocolitis (4%), and thrombosis (2%). The main morbidities were retinopathy (14%), bronchopulmonary dysplasia (10%), and neurodevelopmental deficit (14.5%).ConclusionsSurvival and outcomes of twin to twin transfusion syndrome patients are related to the Quintero stage and gestational age. Therefore, early detection and treatment improve both prognosis and quality of life.
