Pneumonie organisée associée à une cirrhose biliaire primitive

Abstract

Resume Introduction La cirrhose biliaire primitive est une maladie hepatique auto-immune au cours de laquelle les manifestations respiratoires sont rares. Observation Une patiente de 40 ans, suivie depuis cinq ans pour cirrhose biliaire primitive, presentait deux ans avant son hospitalisation une toux chronique et une dyspnee persistante. L’examen clinique trouvait une patiente avec febricule, ictere et râles crepitants aux bases pulmonaires. La radiographie thoracique montrait des opacites alveolaires bilaterales, migratrices, lentement extensives et persistantes. Les investigations microbiologiques etaient negatives. Le bilan biologique objectivait un syndrome inflammatoire, une legere hyperleucocytose sans hypereosinophilie et une hepatite mixte. La recherche des ANCA et des facteurs antinucleaires etait negative. La bronchoscopie montrait une inflammation bronchique diffuse et le lavage bronchoalveolaire une formule cellulaire mixte. Le scanner revelait des condensations pulmonaires peribronchiques avec epaississement de la plevre et des septa interlobulaires evoquant une pneumonie organisee. Sous corticotherapie orale, une amelioration clinique precoce et radiographique plus progressive etaient notees. Un traitement de six mois etait acheve avec un suivi post-therapeutique de six mois sans rechute. Conclusion Cette observation souligne l’interet d’evoquer le diagnostic de pneumonie organisee en cas de cirrhose biliaire primitive associee a une pneumopathie trainante.
