Le dépistage du cancer du poumon

Abstract

Resume L’etude NLST a demontre chez plus de 53 000 grands fumeurs ou anciens grands fumeurs que le depistage annuel par scanner faiblement dose diminue de 20 % la mortalite specifique par cancer bronchopulmonaire et de 6,7 % la mortalite globale. Neanmoins, plusieurs inconvenients potentiels de ce depistage par scanner ont ete regulierement mis en avant, tels que le risque de surdiagnostic, l’irradiation liee aux scanners, et surtout le nombre eleve de faux positifs pouvant generer des examens invasifs et potentiellement dangereux, alterer la qualite de vie et l’anxiete et majorer les couts. Tous ces obstacles theoriques a l’implantation du depistage ont ete abordes dans de nombreuses publications recentes qui concluent pour la plupart a un rapport benefice/risque favorable au scanner. La plupart des societes savantes americaines recommandent de fait ce depistage. En France, un groupe d’experts s’est egalement prononce favorablement en recommandant ce depistage aux fumeurs ou anciens fumeurs de 55 a 75 ans ayant fume au minimum 30 paquets/annees. Ce groupe suggerait egalement la realisation d’etudes cliniques pour preciser et adapter la mise en œuvre de ce depistage aux contraintes et particularites francaises. La Haute Autorite de sante a toutefois juge que la mise en place de ce depistage en France etait prematuree.
