Abstract

Abstract Objectives: To highlight clinical features and management of a case of ceruminous adenocarcinoma of the external auditory canal. To discuss the etiopathogenesis and different treatment modalities available. Methods: Ceruminous adenocarcinoma of the external auditory canal is a rare tumor. It presents with otalgia, ear blockage, and discharging ear. Diagnosis is made by otoscopy and otomicroscopy. The extent of tumor is assessed by imaging with HRCT. Treatment is planned according to the extent of involvement. It is mainly surgical excision of the tumor with wide margins and histopathological confirmation of the nature of tumor. A case of ceruminous adenocarcinoma in a 60-year-old male patient that was successfully managed by excision biopsy with complete healing in 6 weeks is presented along with the histopathological features of the tumor. Results: Ceruminous adenocarcinoma is a rare tumor. A 60-year-old male patient who presented with a painful lesion in the left ear canal which on otoscopy showed a mass 2cm × 1.5cm,in the cartilaginous meatus involving the floor and posterior wall. Audiometry showed mild conductive hearing loss. HRCT confirmed a tumor that was not involving bone. Surgical excision was performed under general anesthesia using endaural approach. Postoperative recovery was uneventful. Histopathology confirmed ceruminous adenocarcinoma of intermediate grade. No further treatment was offered and patient is asymptomatic with no evidence of disease at 18 months follow-up. Conclusion: A rare case of ceruminous adenocarcinoma of external auditory canal is presented. Successful outcome after surgery is highlighted. Importance of follow-up to detect recurrences is stressed.
