Abstract

Mujer de 74 anos sin antecedentes de interes, que acude al servicio de urgencias tras un traumatismo craneoencefalico. En el estudio no se visualizan signos de hemorragia intra o extraaxial ni lineas de fractura. Se aprecia edema en las partes blandas frontales, un minimo hematoma subgaleal y pequenas burbujas de gas con extension intraorbitaria (extraconal). De manera incidental se visualiza una lesion osea pediculada de aproximadamente 11,1 mm (figs. 1 y 2), con origen en el borde superior del poro acustico izquierdo (fig. 3), la cual presenta una morfologia que recuerda a una «seta» (figs. 1 y 2). El nucleo de la lesion esta compuesto por hueso esponjoso, y la periferia por hueso compacto, ambos en continuidad con el hueso del penasco. El resto del canal auditivo interno muestra un calibre y morfologia normal. Estos hallazgos son compatibles con un osteoma del conducto auditivo interno. El principal diagnostico diferencial debe realizarse con la exostosis. A diferencia del osteoma del caso expuesto, esta entidad suele mostrarse como multiples lesiones bilaterales, con base de implantacion ancha, que estan compuestas en su totalidad por hueso compacto, y que de manera secundaria disminuyen la luz del conducto auditivo interno. La clinica descrita asociada al osteoma del poro acustico esta en relacion con alteraciones auditivas y del equilibrio, aunque pueden ser asintomaticos. En funcion de esta sintomatologia se valora el tratamiento mas adecuado o unicamente control evolutivo.
