Abstract

Introduction Cette etude portant sur 26 patients a pour objectif d’evaluer la contribution des avis du nephrologue, du geriatre et du patient, dans la decision de dialyser ou pas. Methodes Âge > 75 ans, DFG Resultats obtenus ou attendus Au total, 26 patients, 16H/11F, âge moyen de 86,7 ans, score moyen de Charlson 7,7 ; troubles cognitifs chez 60 % et absence d’autonomie chez 65,4 % des patients. Groupe C : 6, groupe D : 17, et groupe I : 3. Une famille s’est opposee au choix du patient. Concordance des avis : groupe C = 0 % avec nephrologues 67 % avec geriatres, groupe D = 94 % avec nephrologues, 71 % avec geriatres. L’avis des nephrologues different de celui des patients (p0,006) contrairement aux geriatres. Les avis des deux specialistes ont concorde dans 25 % pour le traitement conservateur et 61 % pour la dialyse (p0,08). Ni l’autonomie ni la depression ni le MMSE Conclusion Malgre ce faible echantillon, l’avis du geriatre semble concorder plus frequemment avec le souhait du patient et de sa famille. Il serait souhaitable de les associer dans la decision du choix therapeutique de l’insuffisance renale terminale, faut-il des RCP a l’avenir ?
