Le PITOLISANT Wakix™ : un nouveau traitement dans la narcolepsie

Abstract

Resume Le Wakix (pitolisant) est un nouveau traitement issu de la recherche francaise pour les patients narcoleptiques. Son mecanisme d’action est original, il s’agit d’un antagoniste/agoniste inverse des recepteurs H3 a l’histamine, il exerce donc un effet pro-eveillant et pro-cognitif par l’intermediaire de ce systeme principalement. Sa posologie initiale est de 9 mg et peut etre augmentee progressivement sans depasser 36 mg, elle se fait en une prise par jour, car sa demi-vie d’elimination plasmatique est de 8 h–11 h. La prescription est reservee aux medecins experimentes dans les troubles du sommeil. Son efficacite sur la somnolence diurne excessive est comparable a celle du modafinil a des posologies entre 100 et 400 mg. L’effet sur les cataplexies reste a preciser. Les effets indesirables les plus frequents etaient : cephalees (8 %), insomnie (6 %), depression (2 %) et anxiete (1,57 %).
