Abstract

ResumenIntroducciónLa oftalmoplejía internuclear bilateral es un síndrome producido por una lesión en el fascículo longitudinal medial, es muy característica de la esclerosis múltiple aunque puede tener etiología vascular, tumoral o infecciosa.Presentación del casoSe presenta el caso de oftalmoplejía internuclear bilateral resuelta 3 meses después del inicio del cuadro como resultado de un proceso isquémico a nivel pontino.ConclusionesLa oftalmoplejía internuclear de etiología isquémica debe ser considerada en el diagnóstico diferencial cuando nos encontramos frente a pacientes con factores de riesgo cardiovascular como obesidad, sedentarismo, dislipidemia, hipertensión, tabaquismo. La resonancia magnética es el método ideal de correlacionar la clínica con la lesión anatómica.AbstractIntroductionInternuclear bilateral ophthalmoplegia is a syndrome caused by a lesion in the medial longitudinal fasciculus, is very characteristic of multiple sclerosis but may have vascular or tumoral etiology.Case presentationWe report the case of bilateral internuclear ophthalmoplegia resolved three months after the onset of the ischemic process resulting at bulb level.ConclusionsIschemic internuclear ophthalmoplegia should be considered in the differential diagnosis when we are faced with patients with cardiovascular risk factors such as obesity, sedentary lifestyle, dyslipidemia, hypertension, smoking. MRI is the best way to correlate the clinical with anatomical lesions.
