Abstract

ResumenEn los pacientes vitrectomizados con aceite de silicona, cirugía refractiva previa y catarata necesitan ajustes biométricos y utilizar factores de corrección con el fin de obtener queratometrías y longitudes axiales reales.ObjetivoEvaluar el comportamiento de las fórmulas de cuarta generación para el cálculo de la lente intraocular (LIO) en estos pacientes.Material y métodoSe realizó un estudio prospectivo, descriptivo, en 6 ojos operados de catarata, con antecedente de cirugía refractiva corneal sin datos anteriores y vitrectomía con aceite de silicona.ResultadosEl promedio de edad fue de 49 años. La longitud axial media fue de 26,79mm. Entre ambos métodos queratométricos hubo una diferencia significativa de más de 1D. El poder medio de la LIO por el IOL Master y la fórmula de Haigis fue de 17,28D, que se modificó utilizando el método de Maloney a 19,28D; con una diferencia media de 2,0D. El componente esférico con aceite de silicona en cavidad, fue de +5,50 D, y cuando se retiró el aceite de la cavidad vítrea posterior a la cirugía de catarata, se obtuvo una esfera promedio de –1,00D, resultados que se consideraron dentro de la emetropía; con el uso de gas, el componente esférico fue de –0,50D. Hubo una mejoría significativa con corrección de 2 líneas en la escala de Snellen.ConclusiónLa correlación entre la fórmula de Haigis y el método de Maloney permite obtener una buena predictibilidad en el cálculo del LIO en pacientes vitrectomizados con aceite de silicona, cirugía refractiva previa y catarata.AbstractThe preoperative studies in patients with previous refractive surgery, cataract and vitrectomy with oil silicone are very difficult to get a good IOL calculation, needs biometric adjustments and the use of correcting factors with the aim of obtaining keratometries, axial real lengths.ObjectiveEvaluate the behavior of fourth generation formulas for the calculation of the IOL power in patients vitrectomized with oil of silicone and previous refractive surgery.Material and methodA prospective and descriptive study was done in 6 eyes operated of cataract, and refractive corneal surgery without previous data and vitrectomy with oil of silicone.ResultsThe average age was 49 years. The average of axial longitude was 26.79mm. Between both keratometric methods there was a significant difference of over 1 diopter. The mean power of IOL for the IOL Master's and the formula of Haigis was of 17.28D, which was modified using Maloney's method to 19.28D; with a half difference of 2.0D. The spherical component with oil of silicone at cavity was of +5,50D; when the posterior vitreous cavity oil was withdrawn to the surgery of cataract, the average sphere obtained was of –1.00D proven to be that they considered themselves within emetropia. With the use of gas, the spherical component was of –0.50D. There was a significant improvement with correction of 2 lines in the scale of Snellen.ConclusionThe relationship between Haigis's formula and the Maloney's method let us to get a good prediction about the IOL calculation in vitrectomized patient with silicon oil, refractive surgery and cataract.
