Abstract

Fundamento y objetivo El control de la hiperglucemia en salas de hospitalizacion convencional es suboptimo. Nuestro objetivo fue evaluar la eficacia en el control glucemico de una intervencion proactiva apoyada por un sistema de point-of-care con visualizacion remota de glucemias capilares (GC), respecto al abordaje habitual en una sala de hospitalizacion de pacientes quirurgicos no-criticos. Pacientes y metodo Se definieron 2 periodos secuenciales de 2 meses de duracion. En la primera fase (control, FC), el propio equipo quirurgico se encargo del control glucemico, se registraron las GC mediante el sistema StatStrip® y se presto soporte por endocrinologo a peticion del cirujano. En una segunda fase (intervencion, FI), un endocrinologo actuo en funcion de los valores de GC visualizados de forma remota. Se compararon el uso de pautas basal-bolo y el grado de control glucemico entre ambos periodos. Resultados La FI se asocio a mayor uso de pautas basal-bolo (21,4 frente 58,3%; p = 0,003). La glucemia media (DE) de la FC fue de 161 (64) mg/dl y la de la FI de 142 (48) mg/dl (p < 0,001). La FI se asocio a mayor frecuencia de GC 70-140 mg/dl (FC 41,8 frente a FI 52,5%; p < 0,001), menor frecuencia de GC = 250 mg/dl (FC 9 frente a FI 3,5%; p < 0,001), sin un aumento del numero de valores en hipoglucemia (FC 3,0 frente a FI 3,7%; p = 0,39). Conclusiones La intervencion proactiva endocrinologica facilitada por un sistema de point-of-care con visualizacion remota de GC se asocia a mejor control glucemico en areas no-criticas, sin un aumento adicional del numero de hipoglucemias registradas.
