Aneurisma gigante de la arteria basilar en un varón joven

Abstract

Varon en su cuarta decada de vida, fumador activo, sin otros antecedentes medicos destacables. Ha consultado al Servicio de Urgencias por cuadro clinico, de instauracion subita, de cefalea hemicranea izquierda y alteracion del lenguaje. La exploracion neurologica ha mostrado paralisis de III y VII pares craneales izquierdos, y ataxia. Se ha realizado angiotomografia computarizada cerebral (figs. 1 y 2), que ha demostrado un aneurisma gigante de la arteria basilar (27 15 mm) con efecto compresivo sobre el mesencefalo, e hidrocefalia asociada, pero sin signos de hemorragia subaracnoidea. Se ha aplicado un clip quirurgico con buen resultado, pero con persistencia de moderada inestabilidad de la marcha. En la busqueda de una posible etiologia se ha realizado estudio autoinmunitario y serologico completo, que no ha resultado concluyente. Tambien se ha realizado ecocardiografia transtoracica, sin evidenciar signos de endocarditis. Aunque el aneurisma o dolicoectasia basilar es raro y no tiene etiologia definida, se considera que la mayoria de los casos son secundarios a procesos ateroscleroticos con mas manifestaciones isquemicas por compresion que lesiones hemorragicas, como es el caso que presentamos. Sin embargo, en pacientes jovenes la coexistencia de anomalias congenitas locales es lo mas aceptado. Figura 2. Figura 1. ww w.els evier .es /med i c in ac l in i c a
