Colistina frente a tobramicina nebulizadas en el tratamiento de la colonización crónica por Pseudomonas en pacientes con fibrosis quística

Abstract

Resumen Fundamento y objetivo Comparar la eficacia, seguridad y satisfaccion del regimen con tobramicina frente a colistina inhaladas en el tratamiento de la colonizacion cronica por Pseudomonas en pacientes con fibrosis quistica. Pacientes y metodo Estudio cuasiexperimental abierto de datos emparejados en el que se incluyo a aquellos pacientes con diagnostico de fibrosis quistica y colonizacion cronica por Pseudomonas aeruginosa. Los pacientes realizaron 2 periodos de 6 meses consecutivos de tratamiento antibiotico nebulizado: tobramicina inhalada a dosis de 300 mg/12 h (Pari LC Plus®) y colistimetato (I-neb®) a dosis de 1 MUI 2 veces al dia. Se realizaron visitas mensuales y, al principio y final de cada periodo, se evaluaron el indice de masa corporal, la funcion pulmonar, el numero de exacerbaciones y la microbiologia del esputo. La satisfaccion del paciente se valoro con 2 cuestionarios de satisfaccion, sobre el tratamiento y sobre el dispositivo de inhalacion. Resultados Se incluyeron 25 pacientes (8 varones y 6 menores de 14 anos) con una edad media (DE) de 21,4 (7,2) anos y un volumen espiratorio forzado en el primer segundo (FEV1) medio del 59,8 (21,1) %. Durante el periodo 2 se obtuvieron beneficios clinicos en terminos de funcion pulmonar, con ganancia de FEV1 de un 5% a los 6 meses de tratamiento y reduccion del numero de exacerbaciones (media de 1,84 [1,2] frente a 0,8 [0,8]; p = 0,001). La puntuacion de los cuestionarios fue superior en el periodo 2. Conclusiones El colistimetato de sodio administrado a traves del sistema de nebulizacion I-neb® aporta beneficios clinicos en terminos de exacerbaciones y de funcion pulmonar, mejorando la satisfaccion del paciente hacia el tratamiento antibiotico nebulizado.
