Abstract

Resumen Introduccion La isquemia arterial aguda se define como un cuadro sindromico debido a una ausencia mas o menos subita de flujo arterial en un territorio concreto. Etiologia y fisiopatologia La etiologia de la isquemia aguda se puede resumir en dos procesos fisiopatologicos: embolicas y tromboticas. Una embolia es el resultado de la migracion de un material, habitualmente desde el corazon, hasta una arteria en un territorio diferente. La trombosis suele ser secundaria a una obstruccion en una arteria por una alteracion basal subyacente en dicha arteria, habitualmente una placa de ateroma. Clinica y diagnostico El cuadro clinico de la isquemia aguda se resume a las «5 p» clasicas: pain (dolor), palidez, parestesias, paralisis y pulseness (ausencia de pulsos). La arteriografia sigue considerandose el gold standard en el diagnostico de la isquemia aguda. La angio-TAC esta cobrando especial importancia debido a la mayor disponibilidad. Tratamiento El tratamiento inmediato consiste en la hidratacion adecuada del paciente y en la anticoagulacion sistemica, que debe instaurarse en cuanto se haya hecho el diagnostico. El tipo de tratamiento quirurgico dependera de la causa de la isquemia.
