Abstract

Resumen Introduccion La hemorragia digestiva baja es una entidad frecuente en nuestro medio, en especial en pacientes anosos y con multiples comorbilidades. Puede presentarse desde en forma leve y autolimitada hasta ocurrir un sangrado masivo con shock hemorragico. Etiopatogenia Su origen habitual es el colon, y la causa mas frecuente son los diverticulos y las angiodisplasias. Diagnostico Disponemos de varias tecnicas diagnosticas y terapeuticas para su manejo, entre las que destacan la colonoscopia, con capacidad diagnostica y terapeutica, la angio-TC multidetector, rapida y sin necesidad de preparacion, y la arteriografia, con una aplicacion habitualmente terapeutica. Tratamiento La cirugia actualmente queda relegada a un tratamiento de rescate cuando no se ha logrado controlar el sangrado por otros medios. Hemorragia gastrointestinal de origen oscuro Se define como aquel sangrado gastrointestinal cuyo origen no se observa tras un estudio convencional. Su causa mas frecuente son las angiodisplasias de intestino delgado, y puede aparecer como una hemorragia manifiesta u oculta. Las pruebas diagnosticas mas utilizadas son la video-capsula endoscopica, la enteroscopia y estudios radiologicos.
