Hemangioma cavernoso sublingual

Abstract

Los angiomas son lesiones vasculares, localizadas y benignas. El hemangioma profundo o cavernoso se localiza bajo el tejido celular subcutaneo, pudiendo localizarse en cualquier parte del organismo; si bien, el 65% de los hemangiomas se localizan en cabeza y cuello1,2. Los hemangiomas son los tumores no epiteliales mas frecuentes con una incidencia del 30% en las glandulas salivares mayores, siendo las lesiones mas comunes en dichas glandulas durante la ninez e infancia temprana, sobre todo en su forma juvenil3. La parotida constituye la localizacion mas frecuente de los hemangiomas que afectan a las glandulas salivares (81-85%)3–5. La incidencia de los hemangiomas en las glandulas salivares menores es dificil de cuantificar, ya que raramente se biopsian, incluso en los biopsiados es dificil distinguir si proviene de una glandula salivar menor o del tejido adyacente, ya que muchas veces se entremezclan ambos. En nuestro caso, histologicamente se observan acinos glandulares entremezcados con el tejido vascular, no pudiendo afirmar ni excluir el origen intraglandular del mismo3,5. Las causas mas comunes de masas en la glandula sublingual y la region submental son: angina de Ludwing, quiste dermoide congenito sublingual, amiloidosis, ranula, quiste branquial, quiste tirogloso y el higroma quistico6, pero si una opacidad esta presente en el area de una glandula salivar, el primer diagnostico a considerar seria sialolitiasis.
