THE PATIENT A 65-year-old, right-handed man presents with a 1.0 1.5 cm erythematous plaque on the dorsal first web space of the right hand. He states that he first noticed the lesion 6 months ago, when it was a smaller, scaly lesion that was about half its current size. No palpable axillary lymph nodes are present. Incisional biopsy reveals squamous cell carcinoma with 25% undifferentiated cells, a Breslow thickness of 1.5 mm, and no subcutaneous extension or perineural invasion.
