Abstract

Resume Introduction La grossesse abdominale (GA) se definit comme la nidation et le developpement de l’œuf feconde dans la cavite peritoneale. Elle est responsable d’une morbi-mortalite materno fœtale grave. Objectif Mettre en exergue les aspects epidemiologiques, diagnostiques, therapeutiques et pronostiques de la GA. Methodologie Etude retrospective realisee a la maternite Josephine Bongo et au centre hospitalier de Libreville de janvier 1999 a decembre 2009 portant sur 19 cas de grossesses abdominales. Resultats La frequence de la GA a Libreville est de un pour 4447 accouchements (0,2 ‰) et une pour 141 grossesses extra-uterines (GEU) tubaires (0,7 %). L’âge moyen etait de 30,5 ± 7,2 ans et la parite moyenne de 2,7 ± 1,7. Le terme moyen de survenue etait 24,3 semaines d’amenorrhee (SA) avec des extremes a 14 et 39 SA. Le diagnostique etait pose devant des douleurs abdominopelviennes chez toutes les patientes et une amenorrhee dans 11 cas (57 %). Il etait confirme par l’echographie dans 14 cas (73,7 %) et apres laparotomie chez cinq (26,3 %) patientes. Nous avons eu deux (5,26 %) naissances vivantes a 39 et 38 SA avec des poids de naissance respectifs de 2380 et 2550 g. La delivrance etait complete dans sept (36,8 %) cas. Cinq (26,3 %) patientes ont presente des complications hemorragiques. Conclusion La GA est une pathologie rare. Son diagnostic est difficile au-dela du second trimestre avec un pronostic fœtal souvent pejoratif.
