Abstract

Os linfomas podem ser classificados em linfomas não-Hodgkin e do tipo Hodgkin, eles são uma proliferação neoplásica de um dos tipos das células do tecido linfopoiético-reticular. Se caracterizam por proliferações clonais de linfócitos T, linfócitos B (mais frequentes) ou de células reticulares os linfomas não-Hodgkin (LNH). Já o linfoma de Hodgkin, que é o objeto deste resumo, também chamado de doença de Hodgkin, é visto como um linfoma maligno, pelo fato de se classificar como, uma doença proliferativa dos tecidos linfoides. A classificação e definição de estadiamento dos linfomas de Hodgkin mais utilizada atualmente é a rotina de estadiamento proposta na conferência de RYE, que classifica a doença em quatro estágios. O linfoma de Hodgkin apresenta como elemento patognomônico a célula de Reed-Sternberg, além de grande variedade de aspecto histopatológico, sendo classificado em quatro tipos, de acordo com Rye: Predominância linfocitária (PL), Esclerose nodular (EM), Celularidade mista (CM) e Depleção linfocitária (DL). Porém, alguns casos não se enquadram em nenhum dos quatro tipos clássicos, ficando inclassificáveis. A doença de Hodgkin afeta principalmente adultos jovens ou de meia idade e incide mais nos indivíduos do sexo masculino e da raça branca. É rara na infância, mas por volta da puberdade, a incidência aumenta rapidamente, e, sobre sua etiologia, têm-se algumas teorias mais aceitas atualmente, como, por exemplo, a proliferação do clone maligno, história anterior a algumas patologias relacionadas à imunodeficiência e alterações cromossômicas. A queixa inicial mais frequente é um aumento progressivo não doloroso de um ou mais linfonodos periféricos geralmente no pescoço, mas o paciente pode não apresentar nenhuma outra queixa ou pode ter sintomas sistêmicos referentes a doenças – febre, sudorese noturna ou perda de peso; às vezes se descobre linfadenopatia, hepatomegalia, esplenomegalia ou uma associação desses achados no exame físico. Os principais exames laboratoriais realizados são: a biópsia de linfonodo, biópsia de medula óssea, hemograma, entre outros. Dependendo do estágio, a doença tem cura, e seu tratamento baseia-se em quimioterapia, radioterapia e no transplante de medula óssea.
