Abstract

s p g s a b s o c m w t t Case presentation We present the case of a 78-year-old man undergoing aortic valve replacement for aortic stenosis. He had no history of tachyarrhythmia or preexcitation and had preserved left ventricular function with mild left ventricular hypertrophy. His postoperative course was complicated by atrial fibrillation and an intermittent wide complex tachycardia. Figure 1 is a 12-lead electrocardiogram (ECG) taken preoperatively, and Figure 2 is a 12-lead ECG taken day 3 postoperatively on the critical care unit. Figure 3 is a 12-lead ECG taken during a typical episode of tachycardia. What is the mechanism of the tachycardia?
