Abstract

Resume La neurofibromatose de type 1 est la plus frequente des phacomatoses et des neurofibromatoses, avec une incidence de 1/2500 naissances. Transmise sur le mode autosomique dominant, elle se traduit par la presence de tumeurs principalement cutanees, cerebrales et des nerfs peripheriques, et par des dysplasies osseuses et vasculaires. Les lesions cutanees les plus caracteristiques de la maladie sont les taches cafe au lait et les neurofibromes cutanes. Les neurofibromes de topographie interne peuvent entrainer diverses complications, la plus grave etant la degenerescence en tumeur maligne des gaines nerveuses peripheriques, qui est la principale cause de mortalite precoce de la maladie. Le gliome des voies optiques, observe chez l’enfant, est un astrocytome pilocytique peu evolutif. Les hamartomes cerebraux sont frequents, benins, et disparaissent apres l’âge de 20 ans. Les scolioses dystrophiques, souvent associees a des neurofibromes et a une dysplasie durale, provoquent souvent des complications orthopediques et neurologiques. Les dysplasies vasculaires entrainent des stenoses arterielles progressives, principalement cerebrales et renales. La severite de la maladie est variable selon les patients et imprevisible. L’imagerie a un role important dans le diagnostic et le suivi des complications de la maladie, qui surviennent pour la plupart dans l’enfance.
