Abstract

Puntos para una lectura rapida • Una reaccion adversa a alimentos (RAA) es “cualquier respuesta clinicamente anormal que pueda atribuirse a la ingestion, contacto o inhalacion de un alimento, de sus derivados o de algun aditivo que estos contengan”. • Alergia e intolerancia alimentaria son conceptos diferentes englobados dentro de las RAA. La intolerancia se diferencia de la alergia alimentaria en que esta producida por un mecanismo no inmunologico, generalmente produce sintomas digestivos y no tiene el mismo riesgo de producir un shock anafilactico. • El diagnostico de intolerancia alimentaria (IA) se basa en una historia clinica detallada con una anamnesis dirigida a los supuestos alimentos intolerantes, exploracion fisica, pruebas complementarias para descartar organicidad y pruebas especificas segun la intolerancia. • La intolerancia a la lactosa (IL) es la intolerancia alimentaria mas frecuente y puede originarse por causas secundarias o primarias. Se sospecha ante la presencia de sintomas digestivos tras la ingesta de lactosa y para su diagnostico pueden usarse diferentes pruebas; la mas utilizada es la prueba de aliento. • El tratamiento de la IL de origen primario consiste en reducir/eliminar la lactosa hasta que desaparezcan los sintomas, pero como la IL es dosis-dependiente, la reduccion del contenido de lactosa puede ser suficiente sin tener que evitar completamente los lacteos. • Existen otras intolerancias menos frecuentes como la IA a la fructosa, a los carbohidratos de cadena corta (FODMAP) o el deficit de glucosa-6-fosfato deshidrogenasa. • Otros probables sindromes de IA son la intolerancia a la histamina y la originada por aditivos y colorantes alimentarios. • El medico de familia debe pensar en esta entidad para poder diagnosticarla y derivar al paciente para la realizacion de pruebas especificas si precisa. Debe realizar el seguimiento y dar informacion veraz a sus pacientes sobre pruebas diagnosticas validadas.
