Tratamiento de las verrugas vulgares y de los condilomas acuminados

Abstract

Puntos clave • Las verrugas estan causadas por la infeccion por el papilomavirus humano (PVH). Se transmiten a traves del contacto directo. El riesgo es mayor en caso de microtraumatismos. • Los tratamientos mas utilizados son la crioterapia y los queratoliticos. Los resultados mejoran si se usan combinados. • Por lo general se requieren multiples sesiones. El fracaso terapeutico no es infrecuente. La crioterapia es dolorosa. • Las verrugas pueden autorresolverse en meses o algunos anos. Un 30% de los casos son cronicos (tolerancia del sistema inmunitario innato por convivencia con el virus desde tiempos ancestrales). • Un elevado porcentaje de las parejas de pacientes con condilomas pueden tener infeccion subclinica. • A todo paciente con condilomas se le ha de aplicar el protocolo de seguimiento de una infeccion de transmision sexual: a) interrogatorio dirigido y exploracion general, oral y anogenital detallada; b) solicitar serologias de sifilis, virus de la inmunodeficiencia humana, virus de las hepatitis C y B; c) localizacion de contactos de los ultimos 6 meses, y d) informar sobre medidas preventivas (evitar conductas de riesgo y usar siempre el preservativo). • Los PVH serotipos 16 y 18 se han relacionado con el cancer anogenital, en especial carcinoma de cervix (hacer un examen ginecologico anual con test de Papanicolau). • A los pacientes con condilomas se les recomienda abstinencia sexual hasta unos 3-6 meses despues de curadas las lesiones (aunque no esta del todo claro si el paciente es contagioso una vez curado clinicamente).
